Job Description

Enhance patient care and independence by making a meaningful impact in the community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity gives you the autonomy to create your own schedule, supporting Medicare Advantage plan members across Perry Township and greater Columbiana County, OH. Each visit takes about 45 minutes and could encompass either annual health evaluations or post-discharge follow-ups, allowing for diverse, engaging interactions every shift.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule
  • Commit to a minimum weekly or bi-weekly number of hours that fits your lifestyle

Primary Responsibilities:

  • Complete annual home health risk assessments on Medicare Advantage members
  • Gather and evaluate patients full medical and medication histories
  • Educate patients about their medical conditions and prescribed medications
  • Identify and report any care gaps with specific recommendations for improvement
  • No treatment plan creation or medication prescribing required

Required Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Six months or more recent NP experience preferred
  • MSN degree from an accredited nursing school
