Responsibilities:
- Conduct comprehensive annual home health risk assessments (approximately 45 minutes per visit)
- Evaluate medical and medication histories to identify health risks and gaps in care
- Educate patients on medical conditions and their medications
- Provide recommendations to resolve identified care gaps
- No treatment plan creation or medication prescribing required
Qualifications:
- Valid Nurse Practitioner License in Ohio
- Current Basic Life Support (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of six months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
- Flexible shifts available Monday through Sunday
- Self-designed schedule with minimum weekly or bi-weekly commitment
