Job Description
Deliver high-quality patient care as a Nurse Practitioner in a dynamic home health environment focused on risk assessment for Medicare Advantage members. This per diem opportunity provides complete scheduling autonomywork as much or as little as your availability allows. Shifts are available any day of the week, and you can build your work calendar to fit your lifestyle.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduling: create your own work calendar
- Minimum weekly or bi-weekly hourly commitment as desired
Key Responsibilities:
- Perform home health risk assessments (approximately 45 minutes each) for Medicare Advantage members residing in the region, including Pleasant Township and surrounding areas
- Travel to patient homes within Brown County
- Complete thorough evaluations of medical history, existing conditions, and medication profiles
- Educate patients on medical conditions, management, and medications
- Recognize and communicate care needs, identifying gaps and providing recommendations
- No treatment planning or prescribing required
Qualifications:
- Nurse Practitioner license, valid in OH
- Graduate of an accredited nursing program (MSN required)
- National board certification (AANP or ANCC)
- CPR certification (BLS)
- At least 6 months of recent NP experience (preferred)
Candidates must be able to travel within the county for home visits. In return for your expertise and compassion, this role offers competitive compensation of $80$90 per completed evaluation, along with a mileage stipend and reimbursement for administrative time.
