Job Description
Take on an impactful per diem role conducting home health risk assessments for Medicare Advantage plan members in Miami County, OH. As a Nurse Practitioner, you will visit members in their homes to evaluate their health status and support their ongoing wellness. These 45-minute assessments take place in Troy, Piqua, Tipp City, and surrounding communities, focusing on both annual and post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of hours weekly or bi-weekly, as your availability allows
Key Responsibilities:
- Complete comprehensive health risk assessments for Medicare Advantage members
- Review prior and current medical history, chronic and acute medical conditions, and medication lists
- Provide individualized education on health conditions and prescribed medications
- Identify gaps in care, offering actionable recommendations to facilitate member wellness
- Focus on clinical assessmentno requirement for prescribing medications or creating new treatment plans
Qualifications:
- Active Nurse Practitioner (NP) license in Ohio
- Current CPR/BLS certification
- National Board Certification via AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This per diem opportunity includes compensation of $80-$90 per evaluation, with additional mileage and administrative stipends provided.
