Job Description
Experienced Nurse Practitioner (NP) sought for per diem home health risk assessments within Montgomery County, Ohio. This role focuses on conducting thorough evaluations for Medicare Advantage plan members in Dayton, Kettering, Washington Township, and surrounding areas. Flexible scheduling allows you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.
Key Responsibilities:
- Perform annual home health risk assessments lasting approximately 45 minutes
- Review patients prior and current medical histories and medication usage
- Educate patients on their medical conditions and medications
- Identify gaps in care and recommend appropriate solutions
- No treatment plan creation or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
