Deliver high-quality care and make a meaningful impact on patients' lives as a Nurse Practitioner performing home health risk assessments within Summit County. This per diem opportunity offers flexibility and autonomy for experienced Nurse Practitioners who value work-life balance and professional fulfillment.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingbuild your own workload
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in a home setting
- Evaluate comprehensive medical and medication histories
- Educate members on their health conditions and medication management
- Recognize care gaps and provide informed recommendations to improve quality outcomes
- Complete thorough documentationno treatment plan development or prescribing required
Desired Skills & Experience:
- Unrestricted Nurse Practitioner License in Ohio
- MSN from an accredited school of nursing
- Board Certification as NP through AANP or ANCC
- Current BLS/CPR certification
- At least 6 months of recent NP experience preferred
- Excellent interpersonal and time management skills
- Strong commitment to patient-centered care
In this role, youll receive $80$90 per evaluation, including a mileage stipend and compensation for administrative time.
