Job Description

Provide comprehensive home health risk assessments for Medicare Advantage plan members across Darke County, OH, including locations such as Greenville, Versailles, and Arcanum. This per diem role offers flexible scheduling with the opportunity to create your own shifts between Monday and Sunday, making it ideal for clinicians seeking autonomy and variety in their practice.

Responsibilities:

  • Conduct annual home health risk assessments, averaging 45 minutes per visit
  • Review and evaluate patients medical history, current conditions, and medication use
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and recommend appropriate solutions
  • Visits involve assessments only; no treatment plans or medication prescribing is required

Qualifications:

  • Nurse Practitioner license valid in Ohio
  • Board certification through AANP or ANCC
  • Current Basic Life Support (BLS) CPR certification
  • Minimum of 6 months of clinical experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Schedule:

  • Flexible shifts with the ability to set your own hours
  • Minimum weekly or bi-weekly commitment
  • Work may take place in any of the designated locations during a shift
