Provide expert care and assessment as a Nurse Practitioner conducting home health risk assessments across Washington County, OH. This per diem opportunity offers flexible scheduling between Monday and Sunday, allowing you to create your own shifts with a minimum weekly or bi-weekly hourly commitment. Your focus will be on Medicare Advantage plan members residing in areas such as Marietta, Belpre, Little Hocking, and Lowell.
Key Responsibilities:
- Perform annual home health risk assessments, averaging 45 minutes each, including post-discharge visits.
- Review and evaluate patients prior and current medical history, conditions, and medication use.
- Educate patients on their medical conditions and medication management.
- Identify gaps in care and provide recommendations to improve health outcomes.
- Note: This role does not require creating treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner License in Ohio.
- Current CPR certification (BLS).
- Board Certification through AANP or ANCC.
- Minimum of 6 months recent NP experience preferred.
- Master of Science in Nursing (MSN) from an accredited nursing school.
Schedule:
- Flexible shifts from Monday through Sunday.
- Ability to build your own schedule to fit your availability.
- Minimum weekly or bi-weekly commitment.
