An opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members throughout Ashtabula County, Ohio. This per diem position offers flexibility, allowing you to create your own schedule with shifts occurring any day Monday through Sunday. Candidates will conduct comprehensive home health risk assessments averaging 45 minutes, including annual health evaluations and post-discharge visits. Assignments may take place in various locations within Ashtabula County such as Conneaut, Ashtabula, or Geneva.
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, medical conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and make appropriate recommendations to improve health outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Licensed Nurse Practitioner in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum six months current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
- Flexible shifts Monday through Sunday
- Self-created schedules to fit your availability
- Minimum commitment required on a weekly or bi-weekly hourly basis
