Job Description

Make a meaningful impact as a Nurse Practitioner specializing in home health assessments. This per diem opportunity offers flexible scheduling and allows you to play a vital role in preventive healthcare for Medicare Advantage members across Columbiana County. You will perform risk assessments in patient homes, with each visit lasting approximately 45 minutes and including locations such as Perry Township, Salem, and East Liverpool. Duties may encompass annual health assessments or post-discharge visits as scheduled.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Set your own schedule
  • Minimal weekly or bi-weekly hour requirements

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Valid CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current NP experience preferred
  • MSN from an accredited nursing program

Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Review medical histories, current conditions, and medication regimens
  • Educate patients about their health conditions and medications
  • Identify gaps in care and suggest practical interventions to enhance health outcomes
  • No requirement for creating formal treatment plans or prescribing medications
