Job Description

Provide comprehensive home health risk assessments as a Nurse Practitioner (NP) working with Medicare Advantage plan members in and around Columbiana County, OH. This per diem role offers flexible scheduling, allowing you to create your own hours between Monday and Sunday with a minimum weekly or bi-weekly commitment. Conducting thorough annual or post-discharge assessments, you will play a vital role in identifying gaps in care and educating patients without the responsibility of creating treatment plans or prescribing medications.

Responsibilities:

  • Perform home health risk assessments averaging 45 minutes per visit within locations including Bucyrus, Galion, and Crestline.
  • Evaluate patients medical histories, current conditions, and medication use.
  • Provide education regarding medical conditions and medication management.
  • Identify gaps in care and recommend appropriate resolutions.
  • Collaborate professionally while maintaining patient confidentiality.

Qualifications:

  • Valid Nurse Practitioner License in Ohio.
  • Current CPR certification (BLS).
  • Board Certification through AANP or ANCC.
  • Minimum of 6 months recent NP experience preferred.
  • Master of Science in Nursing (MSN) from an accredited program.
