Deliver high-quality, patient-centered care while enjoying the freedom to set your own schedule with this per diem Nurse Practitioner opportunity in a dynamic home health setting. As a key member of an interdisciplinary care team, you'll be responsible for conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Brown County, including areas such as Georgetown, Mount Orab, and Ripley.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Complete annual and occasional post-discharge home health risk assessments for Medicare Advantage members.
- Evaluate patients' medical histories, current conditions, and medication regimens.
- Educate patients about their medical conditions and medications to empower proactive care.
- Identify possible gaps in patients' care and provide actionable recommendations.
- Documentation is focused on assessmentsno prescribing or creation of treatment plans required.
Qualifications:
- Active Nurse Practitioner License in the state of Ohio
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC is required
- Minimum of 6 months' Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
