Step into a rewarding per diem role as a Nurse Practitioner delivering high-impact care through home health risk assessments within Ross County. In this position, youll conduct assessments for Medicare Advantage plan members in diverse settings, including annual health reviews and post-discharge follow-ups. Each evaluation takes approximately 45 minutes and offers the opportunity to make a meaningful difference in patient lives across areas such as Scioto, Union Township, and Huntington Township.

- Commit to a minimum weekly or bi-weekly number of hours
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and document comprehensive medical, surgical, and medication histories
- Educate patients about chronic conditions, daily management, and prescribed medications
- Identify gaps in care and recommend solutions, empowering patients to take an active role in their health
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license for Ohio
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience (preferred)
- Current Basic Life Support (BLS/CPR) certification
Experience professional autonomy while supporting vulnerable populations and gaining valuable field experience. In addition to a competitive per evaluation rate between $80$90, this position includes a mileage stipend and compensation for related administrative tasks.
