Job Description

Provide comprehensive home health risk assessments for Medicare Advantage plan members across Hamilton County, OH, including Cincinnati, Forest Park, and Norwood. This per diem opportunity offers flexible scheduling between Monday and Sunday, allowing you to create your own schedule with a minimum weekly or bi-weekly hourly commitment.

Responsibilities:

  • Conduct annual home health risk assessments, averaging 45 minutes per visit, including post-discharge evaluations
  • Review and evaluate medical history, current conditions, and medication regimens
  • Educate patients on medical conditions and medications to support their health management
  • Identify gaps in care and recommend appropriate resolutions
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Licensed Nurse Practitioner in Ohio
  • Current BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Schedule:

  • Flexible shifts Monday through Sunday
  • Create your own schedule based on availability
  • Minimum weekly or bi-weekly hourly commitment
