Seeking an experienced Nurse Practitioner to deliver high-quality home health risk assessments for Medicare Advantage members throughout Pike County, OH. This per diem opportunity offers flexibility and autonomy in your scheduling while making a meaningful impact on patient health outcomes in the community.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule around personal and professional commitments
- Minimum weekly or bi-weekly hourly commitment based on your availability
Key Responsibilities:
- Perform detailed annual health risk assessments in patients homes for a Medicare Advantage health plan
- Review and evaluate prior and current medical history, ongoing medical conditions, and medication use
- Educate members on their medical conditions, medications, and preventive health strategies for wellness
- Identify care gaps and recommend solutions to improve health outcomes
- Focus exclusively on assessment and educationno prescribing or creation of treatment plans required
Required Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of current Nurse Practitioner experience preferred
In this role, you'll receive between $80 and $90 per evaluation completed, along with a stipend for mileage and administrative time. Assignments are typically in home settings throughout Pike County, including Waverly, Pee Pee Township, and Seal Township, providing variety and the chance to serve diverse communities.
