Experienced Nurse Practitioners seeking autonomy and flexibility will thrive in this per diem opportunity providing home health risk assessments throughout Stark County. Working within a Medicare Advantage framework, youll conduct 45-minute in-home evaluations for members in communities like Canton, Massillon, and Alliance. Each visit is tailoredranging from annual health assessments to post-discharge check-insallowing you to make a direct impact without the responsibility for ongoing treatment or prescribing.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive, in-home health risk assessments for Medicare Advantage members
- Review current and past medical history, conditions, and medications
- Deliver clear education on medical diagnoses and medication regimens
- Spot gaps in care, offering actionable recommendations for improved patient outcomes
- No treatment planning or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Current MSN from an accredited nursing school
- Valid CPR (BLS) certification
- At least 6 months of NP experience preferred
