Enhance patient care in your community by conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Stark County. This per diem position is ideal for experienced Nurse Practitioners who are dedicated to promoting preventive care in a home health setting. Each assessment typically lasts about 45 minutes and may include annual health evaluations or post-discharge visits. The service area encompasses Canton, Massillon, Alliance, and the surrounding areas, offering a dynamic and rewarding experience every shift.

- Flexible shifts available Monday through Sunday

- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Evaluate and document patient medical history, current conditions, and medication use
- Provide education about health conditions and prescribed medications
- Identify gaps in care and offer recommendations to promote better patient outcomes
- No responsibility for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner License in Ohio
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited school of nursing
- At least 6 months of recent NP experience preferred
This opportunity offers compensation of $80-$90 per evaluation, along with a mileage stipend and reimbursement for administrative time.
