Job Description

This per diem position offers experienced Nurse Practitioners the opportunity to conduct home health risk assessments within Guernsey County, OH. You will support Medicare Advantage plan members by evaluating their health status during visits that typically last around 45 minutes. Visits may include annual health assessments or post-discharge evaluations, conducted in various locations such as Cambridge Township, Cambridge, and Jackson Township.

Responsibilities:

  • Perform thorough annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical histories, including medical conditions and medications
  • Educate patients on their medical conditions and prescribed medications
  • Identify any gaps in care and recommend appropriate resolutions
  • Documentation of assessment findings; note that treatment plan creation and prescribing medication are not required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) certification
  • Board certification through the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Minimum of six months recent NP experience preferred
  • Master's degree in Nursing (MSN) from an accredited program

Schedule & Location:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with a minimum weekly or bi-weekly hourly commitment
  • Assignments are within Guernsey County, including Cambridge, Cambridge Township, and Jackson Township areas
