Providing comprehensive home health risk assessments for Medicare Advantage plan members, this per diem Nurse Practitioner (NP) opportunity centers in Guernsey County, Ohio. Serving areas including Cambridge Township, Cambridge, and Jackson Township, the role involves flexible scheduling to fit your lifestyle, with shifts available Monday through Sunday and the ability to create your own work schedule. A minimum weekly or bi-weekly hourly commitment is required, making this ideal for NPs seeking flexibility alongside meaningful patient care.

Key Responsibilities:

  • Conduct annual home health risk assessments averaging 45 minutes per visit.
  • Evaluate patients prior and current medical history, medical conditions, and medication history.
  • Educate patients on their medical conditions and medications.
  • Identify gaps in care and provide recommendations for resolution.
  • No treatment plan creation or medication prescribing is necessary.

Qualifications:

  • Active Nurse Practitioner license in Ohio.
  • Current CPR certification (BLS).
  • Board Certification through AANP or ANCC.
  • MSN degree from an accredited school of nursing.
  • Preferably 6 months of recent NP experience.
