Job Description

Schedule:

  • Flexible shifts available seven days a week
  • Design your own schedule for optimal work/life balance
  • Set a minimum weekly or bi-weekly hourly commitment to suit your needs

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage members
  • Review and evaluate current and prior medical histories, medical conditions, and medication use
  • Deliver patient education on health conditions and medication management
  • Identify gaps in care and recommend appropriate next steps for improved outcomes
  • Note: This role does not require developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board certification through AANP or ANCC
  • CPR certification (BLS required)
  • MSN or equivalent from an accredited nursing program
  • At least 6 months of direct NP experience preferred

This position offers a rate of $80$90 per evaluation, supplemented with a mileage stipend and compensation for administrative time.
