Job Description

Step into a meaningful per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. In this flexible, patient-centered role, you will visit Medicare Advantage plan members throughout Summit County, including areas such as Akron, Cuyahoga Falls, Stow, and Gree. Each appointment generally takes around 45 minutes, involving either annual health assessments or post-discharge visits within the comfort of patients homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to design your own workweek
  • Minimum weekly or bi-weekly hourly commitment for ongoing engagement

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and document detailed medical, surgical, and medication histories
  • Assess current and prior health conditions, identifying any care gaps
  • Offer practical education about chronic conditions and prescribed medications
  • Suggest actionable steps to improve patient well-being and resolve identified care gaps
  • No responsibility for prescribing medication or formulating treatment plans

Qualifications:

  • Active Nurse Practitioner license in the state of Ohio
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR certification (BLS)
  • At least six months of recent NP experience is preferred
