Job Description
Deliver critical healthcare services in a flexible per diem role conducting in-home health risk assessments for Medicare Advantage plan members across Preble County, OH, including locations such as Eaton, Camden, and Lewisburg. This position allows you to choose your own scheduleflexible shifts are available Monday through Sunday with minimum weekly or bi-weekly hour commitments.
Schedule:
- Flexible shifts available any day of the week
- Self-scheduling to accommodate your availability
- Guaranteed minimum hourly commitment per week or bi-weekly
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage enrollees
- Review and evaluate prior and current medical history, conditions, and medications
- Deliver patient education relating to medical conditions and medication management
- Identify care gaps and recommend appropriate next steps for resolution
- Visits are approximately 45 minutes in duration and may include annual assessments or post-discharge follow-ups
- No prescribing or treatment planning required; strictly focused on assessment and education
Qualifications:
- Current Nurse Practitioner (NP) license in Ohio
- Board certification through AANP or ANCC
- Active BLS (CPR) certification
- MSN from an accredited nursing program
- At least six months of recent NP experience preferred
