Healthcare professionals with a passion for geriatric medicine and patient-centered care are sought for a rewarding Nurse Practitioner role focused on nursing home and assisted living settings. This contract opportunity centers on providing comprehensive healthcare for insurance plan members, with the primary aim of managing patient needs on-site to minimize unnecessary hospitalizations and encourage safe transitions back to home environments.

Schedule:

• Monday Friday, 8 AM 5 PM
• No weekends or holidays required

Key Responsibilities:

• Conduct monthly (or more frequent as needed) health assessments for residents
• Develop and manage medical problem lists and individualized treatment plans
• Deliver primary, preventative, urgent, and acute care directly within skilled nursing and assisted living facilities
• Identify and provide increased visits to high-risk residents, implementing contingency plans for those at risk for hospitalization
• Communicate and collaborate regularly with each residents primary care physician to ensure cohesive care
• Provide hands-on bedside care, create and update care plans and clinical orders, and work closely with facility staff, directors of nursing, and other healthcare professionals
• Support discharge planning by conducting single visits for residents recently transitioned from hospital stays, facilitating safe transitions out of skilled nursing facilities
• Manage a panel of assigned members across multiple nursing facilities, primarily working at one location daily, with occasional travel to other sites for urgent assessments

Qualifications:

• Masters Degree in Nursing (MSN)
• Active Nurse Practitioner license in New York
• BLS certification required
• Experience in geriatric, skilled nursing, or assisted living environments preferred
