Job Description
A rewarding opportunity is available for a Nurse Practitioner specializing in nursing home and assisted living care. This contract role focuses on delivering comprehensive healthcare to insurance members residing in skilled nursing and assisted living facilities. The primary goal is to provide effective on-site care to reduce unnecessary hospitalizations and support smooth transitions back to home environments.
Located in Brooklyn, NY, this position offers a Monday through Friday schedule from 8 AM to 5 PM, with no weekend or holiday shifts, allowing for a positive work-life balance.
Key Responsibilities:
- Conduct thorough health assessments on residents at least monthly, increasing frequency as needed
- Develop individualized problem lists and medical treatment plans for each resident
- Provide primary, preventive, urgent, and acute care directly on site
- Stratify high-risk patients for more frequent clinical monitoring and develop contingency plans
- Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
- Support transitions by assessing patients newly admitted from hospital discharge and preparing them for safe return home
- Manage a panel of members across multiple facilities, with typical scheduling of one facility visit per day
Qualifications:
- Master of Science in Nursing (MSN) degree
- Valid New York Nurse Practitioner license
- Current Basic Life Support (BLS) certification
