Job Description

This opportunity offers Nurse Practitioners the chance to provide comprehensive, patient-centered care within skilled nursing and assisted living environments. The focus is on delivering high-quality medical assessments and treatments aimed at minimizing unnecessary hospital visits and supporting safe transitions back to home settings.

Schedule/Hours:

  • Monday through Friday
  • 8:00 AM to 5:00 PM
  • No weekends or holidays

Key Responsibilities:

  • Conduct thorough health assessments on residents at a minimum monthly frequency, more often as needed
  • Develop and maintain individualized problem lists and medical treatment plans for each patient
  • Provide primary, preventive, urgent, and acute care directly on site
  • Identify and prioritize high-risk patients, creating contingency plans accordingly
  • Collaborate closely with residents primary care physicians and facility staff including Directors of Nursing
  • Deliver hands-on bedside care, coordinate care plans, and manage clinical orders
  • Support patient transitions by assessing those newly discharged from hospitals to ensure a safe, healthy adjustment in the nursing or assisted living facility
  • Manage a panel of patients across multiple facilities, typically scheduled at one site per day but flexible to visit others as patient needs arise

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • NY Nurse Practitioner license in good standing
  • Basic Life Support (BLS) certification
