Job Description
Make a meaningful impact on the lives of seniors by providing comprehensive healthcare for residents in skilled nursing, assisted living, and independent living settings. In this contract role, your advanced clinical expertise will ensure high-quality, patient-centered care and help reduce unnecessary hospitalizations while supporting smooth transitions back home.
Schedule:
- Monday through Friday, 8 AM to 5 PM
- No weekend or holiday shifts
Key Qualifications:
- Master of Science in Nursing (MSN) degree
- Active Nurse Practitioner license in New York
- Current BLS certification
Role Responsibilities:
- Conduct monthlyor more frequent as neededhealth assessments for patients, with a focus on managing and improving overall healthcare within the facility setting.
- Develop problem lists and implement individualized treatment plans, addressing both acute and chronic conditions.
- Provide a full spectrum of care on site, including primary, preventive, urgent, and acute interventions.
- Identify and closely monitor high-risk patients, increasing clinical visits and establishing contingency plans as necessary.
- Collaborate regularly with residents primary care physicians and facility staff (including Directors of Nursing) to ensure a coordinated approach to care.
- Oversee safe transitions for patients newly discharged from hospitals to nursing facilities, performing on-site assessments and preparing for successful returns home.
- Manage an assigned panel of members across multiple facilities; while typically scheduled at a single site per day, flexibility to respond to acute needs at other facilities is required.
