Job Description

Step into a vital Nurse Practitioner position serving patients in skilled nursing and assisted living settings. This contract role enhances healthcare delivery by providing comprehensive care to insurance members onsite, aiming to reduce unnecessary hospitalizations and foster smooth transitions home.

Schedule:

  • Monday through Friday, 8 AM 5 PM
  • No weekends or holidays required

Key Responsibilities:

  • Conduct regular health assessments for residents at least monthly, escalating frequency for those identified as high-risk.
  • Develop individualized problem lists and medical treatment plans for each patient.
  • Deliver primary, preventive, urgent, and acute care directly in the facility, with hands-on bedside involvement.
  • Identify and stratify high-risk patients, implementing contingency plans and increasing monitoring as needed.
  • Coordinate and communicate effectively with primary care physicians, directors of nursing, and other facility staff to ensure the highest standard of care.
  • Tailor care plans and create orders to meet the evolving needs of your panel across multiple facilities.
  • Facilitate single visits for newly admitted residents transitioning from hospital discharge, assessing their readiness for independent living and supporting safe transitions out of skilled nursing care.
  • While you will typically provide care at one assigned facility per day, flexibility to respond to acute patient needs at other locations may be occasionally required.

Qualifications:

  • Master of Science in Nursing (MSN) degree required
  • Active Nurse Practitioner license in New York
  • Basic Life Support (BLS) certification
  • Demonstrated clinical experience in nursing home, assisted living, or similar care settings is highly valued

Compensation for this role ranges from $75 to $100 per hour, reflecting the importance of your advanced practice expertise.
