Deliver personalized care and make an impact in the community by conducting home health risk assessments for Medicare Advantage members across Belmont County, Ohio. This per diem Nurse Practitioner opportunity offers maximum schedule flexibility and the autonomy to create your own work routine.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule based on your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual health risk assessments in patients homes, with each assessment lasting approximately 45 minutes
- Complete visits that may vary in focus, including annual health reviews and post-discharge evaluations
- Evaluate patients medical histories, current conditions, and medication regimens
- Educate members on their medical conditions and medication usage
- Identify gaps in care, providing actionable recommendations for improved patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certified through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
