Enjoy autonomy and flexibility as a skilled Nurse Practitioner providing essential home health risk assessments in Meigs County, OH. This per diem position empowers you to create your own schedule with minimum weekly or bi-weekly hour requirements, making it ideal for balancing professional and personal commitments. Shifts are available any day of the weekMonday through Sundayand locations include Salisbury Township, Pomeroy, Rutland Township, Chester Township, and surrounding areas.

Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members.
- Evaluate patients prior and current medical history, including conditions and medication use.
- Deliver clear education to patients regarding their medical conditions and prescribed medications.
- Identify any potential gaps in care and provide actionable recommendations.
- Assessments may include annual health evaluations or post-discharge visits, typically lasting about 45 minutes.
- No development of treatment plans or medication prescribing is required.
Qualifications:
- Active Nurse Practitioner license in Ohio.
- Current CPR (BLS) certification.
- Board Certification from AANP or ANCC.
- MSN degree from an accredited nursing school.
- At least 6 months recent NP experience preferred.
This position offers competitive compensation of $80$90 per completed evaluation, plus additional stipends for mileage and administrative time.
