Job Description

A Nurse Practitioner (NP) position is available conducting home health risk assessments within Huron County, OH. This per diem role offers a unique opportunity to provide essential care evaluations for Medicare Advantage plan members in Norwalk, Bellevue, Willard, and surrounding areas. Flexible scheduling allows you to create your own shifts anytime Monday through Sunday, with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Conduct thorough annual home health risk assessments lasting approximately 45 minutes
  • Evaluate patients medical history, current conditions, and medication usage
  • Educate patients on managing medical conditions and medications effectively
  • Identify gaps in care and recommend appropriate resolutions
  • No treatment plan creation or prescribing medications required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) certification
  • Board Certification through AANP or ANCC
  • Preferred minimum of 6 months recent NP experience
  • Master of Science in Nursing (MSN) from an accredited institution
