Job Description
Deliver comprehensive home health risk assessments for Medicare Advantage members across Erie County, OH, including locations such as Sandusky, Huron, and Vermilion. This per diem position offers flexible scheduling options, allowing you to craft your own workweek with shifts available Monday through Sunday.
Key Responsibilities:
- Conduct annual home health risk assessments, typically lasting about 45 minutes, including post-discharge visits.
- Review and evaluate patients medical history, current conditions, and medication use.
- Provide patient education regarding medical conditions and medication management.
- Identify gaps in care and recommend appropriate resolutions.
- Note: This role does not involve creating treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner license in Ohio.
- Current Basic Life Support (BLS) CPR certification.
- Board Certification through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
- Preferably at least 6 months of clinical experience as a Nurse Practitioner.
- Master of Science in Nursing (MSN) from an accredited institution.
Schedule:
- Flexible shifts between Monday and Sunday.
- Create your own schedule to best fit your availability.
- Minimum commitment required on a weekly or bi-weekly basis.
