Make a meaningful impact by delivering quality healthcare directly to patients in their homes. In this per diem Nurse Practitioner opportunity, youll provide valuable home health risk assessments for Medicare Advantage members throughout Pike County, Ohio. Each evaluation runs approximately 45 minutes and may involve an annual assessment or a post-discharge check-in, offering variety and a strong sense of purpose.

Key Responsibilities:
- Complete comprehensive, annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical histories, medications, and chronic or acute medical conditions
- Educate patients on their health status, medical conditions, and prescribed medications
- Identify gaps in care and offer recommendations for follow-up, without the responsibility of developing treatment plans or prescribing medications
Desired Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited program
- At least 6 months of Nurse Practitioner experience preferred
