Per Diem Nurse Practitioner Home Health Risk Assessments
A valued opportunity exists for a Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members within Highland County, Ohio. This per diem role offers flexible scheduling, allowing you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly commitment. The position involves traveling to patient homes in various locations such as Hillsboro, Greenfield, and Leesburg, providing meaningful healthcare services within the community.
Key Responsibilities:
- Perform comprehensive annual home health risk assessments, approximately 45 minutes in duration, for Medicare Advantage patients.
- Review patients medical history, current health status, and medication use.
- Provide clear education to patients about their medical conditions and prescribed medications.
- Identify gaps in patient care and recommend solutions to improve health outcomes.
- Note that this role does not involve treatment plan creation or prescribing medications.
Qualifications:
- Valid Nurse Practitioner license in Ohio.
- Board Certification through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
- Current Basic Life Support (BLS) CPR certification.
- Master of Science in Nursing (MSN) from an accredited institution.
- At least six months of recent Nurse Practitioner clinical experience preferred.
