Deliver comprehensive home health risk assessments as a Nurse Practitioner (NP) focused on Medicare Advantage plan members in Adams County, OH. This per diem role offers exceptional flexibility with shifts available Monday through Sunday, allowing you to create a schedule that suits your lifestyle. Engage in meaningful patient interactions across locations such as Peebles, West Union, and Manchester.
Key Responsibilities:
- Conduct annual home health risk assessments averaging 45 minutes per visit
- Review and evaluate patients prior and current medical histories, conditions, and medication usage
- Educate patients on their medical conditions and medications to enhance care understanding
- Identify gaps in care and provide recommendations to improve patient outcomes
- Perform assessments without the need for treatment plan creation or prescribing medications
Qualifications and Desired Experience:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
