Provide comprehensive home health risk assessments as a Nurse Practitioner for Medicare Advantage plan members in Ashtabula County, OH. This per diem opportunity offers flexible scheduling with shifts available Monday through Sunday, allowing you to create a work schedule that fits your lifestyle. Assessments typically last about 45 minutes and may include annual health evaluations or post-discharge visits across Ashtabula, Conneaut, and Geneva.
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and recommend solutions for improved health outcomes
- No treatment plan development or medication prescribing involved
Qualifications and Desired Experience:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Current CPR/BLS certification
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
- Flexible shifts Monday through Sunday
- Minimum weekly or bi-weekly hourly commitment
