A Nurse Practitioner opportunity is available to conduct home health risk assessments within Darke County, Ohio. This per diem role offers flexible scheduling between Monday and Sunday, allowing you to create your own schedule with a minimum weekly or bi-weekly hourly commitment. As an NP, you will play a crucial role in evaluating and supporting Medicare Advantage plan members in the Greenville, Versailles, Arcanum, and surrounding areas.
Responsibilities include performing annual home health risk assessments on Medicare Advantage members with visits that last approximately 45 minutes. You will review prior and current medical history, assess medical conditions, and evaluate medication usage. Education on medical conditions and medication management will be provided to patients, and you will identify gaps in care while recommending appropriate resolutions. This role does not require treatment plan creation or prescribing medications.
Key Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum of six months current experience as an NP preferred
