Schedule:

• Flexible shifts available Monday through Sunday
• Create your own custom work schedule
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members in their residences
• Review and evaluate patients medical history, current health conditions, and medication use
• Educate patients on their medical conditions and prescribed medications to encourage understanding and compliance
• Identify care gaps and provide actionable recommendations for improved health outcomes
• Complete required documentationno need for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification required (AANP or ANCC)
• Current CPR certification (BLS)
• Completion of MSN degree from an accredited nursing program
• Minimum six months of recent NP experience strongly preferred

In this position, you can expect compensation of $80-$90 per evaluation, along with a stipend for mileage and administrative time.
