This per diem role offers flexibility and autonomy, enabling you to tailor your shifts to your lifestyle and ensure meaningful patient interactions.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule to suit your needs
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete comprehensive annual health risk assessments in patient homes, typically lasting 45 minutes per visit
• Evaluate patients medical history, chronic conditions, and medication use to ensure thorough documentation
• Deliver patient education on disease management and medications, fostering improved health outcomes
• Identify and communicate care gaps and provide actionable recommendations without the need for creating treatment plans or prescribing medications
• Provide exceptional patient care across Summit County, including Akron and neighboring communities

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification by AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing institution
• Minimum of 6 months recent Nurse Practitioner experience preferred
