Make a meaningful impact by providing comprehensive, personalized care in a home setting as a Nurse Practitioner supporting Medicare Advantage plan members. This per diem role focuses on conducting detailed home health risk assessments throughout Stark County, with flexible scheduling options to fit your availability.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum hourly commitment, either weekly or bi-weekly
Primary Responsibilities:
- Perform annual health risk assessments and post-discharge visits in patients homes for Medicare Advantage plan members
- Thoroughly review and evaluate past and present medical histories, existing and previous conditions, as well as medication regimens
- Offer clear, compassionate education about medical concerns and prescribed medications
- Identify care gaps and provide evidence-based recommendations to optimize member health
- Focus entirely on assessment and educationno treatment planning or medication prescribing required
Desired Qualifications:
- Active Nurse Practitioner License (Ohio)
- Current BLS/CPR certification
- National board certification via AANP or ANCC
- Minimum of 6 months of current Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
You will receive between $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time, recognizing the full scope of your efforts across sites such as Canton, Massillon, and Alliance.
