Job Title: LPN Home Health Travel Contract
Duration: 13 weeks
Hours: 40 hours per week
Location: Muncie, IN

Description:

We are seeking a dedicated and experienced LPN (Licensed Practical Nurse) to join our home health team for a 13-week travel contract. This position offers the opportunity to make a meaningful difference in the lives of patients within the comfort of their own homes.

Responsibilities:

Provide compassionate and skilled nursing care to patients in their homes according to prescribed care plans.
Perform comprehensive assessments and document findings accurately.
Administer medications and treatments as prescribed by the physician.
Monitor patients' condition and progress, reporting any changes to the healthcare team.
Educate patients and their families about their conditions and self-care techniques.
Collaborate effectively with other healthcare professionals to ensure coordinated and holistic care.
Maintain accurate and timely documentation using electronic medical records systems.

Requirements:

Current LPN licensure in Indiana (IN)
Minimum of 6 months of nursing experience, preferably in home health or a similar setting.
Experience with Homecare Home Base software preferred.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Reliable transportation and valid driver's license.
CPR certification (BLS for Healthcare Providers) required.
