Position Type: LPN

Hours: Guaranteed 40 hours per week

Qualifications:

Active LPN license in the state of New York
Active Basic Life Support (BLS) certification with the American Heart Association or the American Red Cross

Responsibilities:

Deliver compassionate and competent nursing care to residents in a long-term care setting
Collaborate with interdisciplinary team members to assess, plan, implement, and evaluate resident care plans
Administer medications and treatments as prescribed, ensuring accuracy and adherence to facility policies and procedures
Monitor residents' health status and respond promptly to changes or concerns
Assist with resident admissions, transfers, and discharges as needed
Provide education and support to residents and their families regarding health conditions, treatments, and care plans
Maintain accurate and thorough documentation of nursing assessments, interventions, and outcomes
