Hours: Guaranteed 40 hours per week

Qualifications:

  • Active LPN license in the state of New York
  • Active Basic Life Support (BLS) certification with the American Heart Association or the American Red Cross

Responsibilities:

  • Deliver compassionate and competent nursing care to residents in a long-term care setting
  • Collaborate with interdisciplinary team members to assess, plan, implement, and evaluate resident care plans
  • Administer medications and treatments as prescribed, ensuring accuracy and adherence to facility policies and procedures
  • Monitor residents' health status and respond promptly to changes or concerns
  • Assist with resident admissions, transfers, and discharges as needed
  • Provide education and support to residents and their families regarding health conditions, treatments, and care plans
  • Maintain accurate and thorough documentation of nursing assessments, interventions, and outcomes
