Job Description

Sunbelt is seeking a Licensed Practical Nurse for a Home Health contract position with a local client near Lexington, KY. Home Health LPNs with at least one year of recent home health experience are encouraged to apply for this opportunity. Responsibilities:

Utilize expert knowledge of Medicare/Medicaid Documentation
Complete accurate and detailed documentation within a 24-48-hour turnaround time
Case manage at least 5-7 patients per day and 25-30 patients per week
Scheduled visits will include, but are not limited to:
- Providing wound care
- IV administration and teaching
- Cardiac program follow up and intervention
Drive an average of 200 miles per week in the Lexington, KY area

Experience:

At least one year of Home Health experience within the last year required
In-depth knowledge and field experience of Medicaid/Medicare
Experience case managing at least 5-7 patients per day and 25-30 per week

Education:

Kentucky Licensed Practical Nurse License
American Heart Association Basic Life Support

Employment Details:

Employment Type: Full-time, Contract
Work Hours: 8:00AM-5:00PM, 40 hours per week, some on-call and weekends required
13 week assignment with possible extension
