Responsibilities:

Administer medications and treatments according to physician orders.
Assist patients with activities of daily living.
Monitor patient health status and report any changes to healthcare team.

Requirements:

Valid LPN license in the state of Wisconsin.
Previous experience in long-term care preferred.
Current BLS certification.
