Job Description
Experienced and compassionate Registered Nurses seeking an opportunity to deliver direct, patient-centered care outside a traditional hospital setting will find this field-based position highly rewarding. For a 13-week full-time contract, youll collaborate with patients and families, offering skilled nursing within their homes and serving as a crucial advocate in their ongoing care journey.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (average of 35 patient visits daily)
- Field-based visits, with daily travel within your assigned territory
Key Responsibilities:
- Conduct thorough OASIS assessments and implement personalized care plans
- Deliver specialized nursing care: wound management, IV therapy, chronic disease education, and medication administration
- Monitor patient status, communicate changes promptly with physicians and healthcare teams
- Educate patients and families on care processes, safety, and medication management
- Maintain accurate documentation within Home Care Home Base (HCHB)
- Coordinate with a multidisciplinary team, including therapists and social workers
- Participate in meetings and ongoing professional development
- Adhere to regulatory and agency policies for documentation and care quality
Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent home health nursing experience
- Demonstrated proficiency in OASIS and Home Care Home Base (HCHB)
- Solid clinical assessment, critical thinking, time management, and organizational abilities
- Reliable transportation, valid drivers license, and auto insurance
- Strong independent work ethic and communication skills
- BSN and experience managing 2532 points per week preferred
- Knowledge of Medicare and home health compliance a plus
