An opportunity is available for a skilled and compassionate Registered Nurse with experience in home health to deliver meaningful, patient-centered care in a field-based setting. This full-time, 13-week contract position focuses on providing personalized nursing services to patients in their homes while supporting consistent care, professional growth, and multidisciplinary collaboration.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
You will conduct around 35 patient visits per day (caseload: 2532 points per week), utilizing the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment tool. Daily travel within your assigned territory is required, and mileage reimbursement is provided.
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing tasks including wound care, IV therapy, medication management, and chronic disease education
- Continuously monitor patient condition, document accurately in HCHB, and communicate proactively with physicians and multidisciplinary teams
- Educate and support patients and families on disease processes and preventive care
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing training
- Adhere to all clinical and regulatory standards for safe, effective care
Qualifications:
- Current RN license in good standing
- Minimum 1 year of recent home health (field) experience
- Proficiency in OASIS and Home Care Home Base (HCHB)
- Reliable transportation with valid drivers license and auto insurance
- Strong organizational and communication abilities, with a high degree of clinical competence
- Preferred: BSN, experience managing 2532 points/week, knowledge of Medicare/compliance
