Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans for home health patients.
- Deliver skilled nursing care such as wound management, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, communicate changes to healthcare providers, and adjust care plans as needed.
- Educate patients and their families about health conditions, treatment plans, medications, and preventive measures.
- Ensure timely, accurate documentation using the Home Care Home Base (HCHB) electronic system.
- Work collaboratively with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care delivery.
- Participate in case conferences, staff meetings, and ongoing education to maintain high standards of care and compliance.
Qualifications:
- Active RN license in good standing (state of practice required).
- At least 1 year of recent field-based Home Health nursing experience.
- Experience with Home Care Home Base (HCHB) and OASIS is mandatory.
- Strong clinical assessment, organizational, and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and efficiently manage a variable field-based caseload (2532 points per week).
- BSN preferred, along with knowledge of Medicare and home health compliance standards.
