Seeking a skilled and compassionate Registered Nurse to provide exceptional in-home care to patients as part of a dedicated home health team. This opportunity is ideal for nurses who are self-driven, organized, and thrive in a field-based, patient-centered environment. As a Home Health RN, you will play a pivotal role in ensuring patients receive high-quality care and support to optimize their health and independence.

Schedule:

• Monday Friday
• 8:00 AM 5:00 PM
• 13-week contract

Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans for each patient
• Deliver skilled nursing interventions such as medication administration, wound care, IV therapy, and chronic disease management
• Monitor patient conditions; communicate critical changes promptly to physicians and care teams
• Educate patients and families about disease processes, treatment, medications, and preventive health
• Document all assessments, interventions, and communications accurately using Home Care Home Base (HCHB)
• Coordinate with an interdisciplinary team, including therapists, social workers, and home health aides
• Participate in meetings, case conferences, and ongoing education sessions
• Daily travel to patient homes within assigned territory (mileage reimbursement provided)

Required Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health experience (field experience preferred)
• Prior experience with OASIS and proficiency using Home Care Home Base (HCHB)
• Strong clinical assessment and critical thinking skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently with strong time management and organizational skills

Preferred:

• BSN
• Experience managing a caseload of 2532 points per week
• Knowledge of Medicare and home health compliance

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
