An opportunity is available for a skilled Home Health Registered Nurse (RN) to deliver compassionate, patient-centered care within the homes of patients. This field-based role involves providing comprehensive nursing services while working independently, maintaining clinical excellence, and fostering strong patient communication. The position requires managing a caseload of 2532 points weekly, typically involving 35 patient visits per day.

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans.
• Administer skilled nursing interventions, including wound care, medication management, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate promptly with healthcare teams and physicians.
• Educate patients and families on health conditions, treatments, medications, and preventive care.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate closely with interdisciplinary teams physical therapists, occupational therapists, social workers, and home health aides to ensure seamless care coordination.
• Participate in case conferences, staff meetings, and continuing education activities.
• Adhere to all regulatory and agency standards governing documentation and patient care delivery.

Qualifications:

• Active RN License in good standing for the state of practice.
• Minimum of 1 year recent Home Health experience, with field experience preferred.
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments.
• Strong clinical assessment, critical thinking, organizational, and communication skills.
• Reliable transportation with a valid drivers license and insurance.
• Ability to independently manage a flexible schedule within patient territories.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance standards.
