Job Description

Provide expert, compassionate care as a Hospice Registered Nurse (RN) delivering end-of-life support to patients in home and residential settings. This full-time contract role spans 13 weeks, offering meaningful field-based work with a focus on comfort, dignity, and quality of life for terminally ill patients and their families.

Key Responsibilities:

  • Perform thorough nursing assessments and develop personalized hospice care plans.
  • Deliver skilled nursing services emphasizing symptom management, pain relief, and patient comfort.
  • Administer medications and treatments per physician orders and hospice protocols.
  • Monitor patient conditions, promptly communicating changes to the medical director and care team.
  • Educate patients, families, and caregivers on disease progression, comfort measures, and emotional support.
  • Collaborate with interdisciplinary teams including social workers, chaplains, and aides to holistically address patient needs.
  • Maintain accurate and timely documentation using Home Care Home Base (HCHB) software.
  • Participate in case conferences, team meetings, and quality improvement initiatives.
  • Adhere to hospice policies, procedures, and regulatory requirements.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum of one year recent hospice nursing experience.
  • Proficient in Home Care Home Base (HCHB) documentation.
  • Strong clinical assessment and communication skills.
  • In-depth knowledge of pain management and end-of-life care principles.
  • Reliable transportation with a valid drivers license and insurance.
  • Ability to work independently managing a field-based schedule.
  • Compassionate, emotionally resilient approach.
  • BSN preferred.
  • Experience with palliative care and Medicare hospice regulations a plus.

Schedule & Location:

  • Monday through Friday, 8:00 AM to 5:00 PM, with occasional weekend or on-call rotation.
  • Approximately 4 to 5 patient visits per day across a defined service area.
  • Daily travel required; mileage reimbursement provided.
