An exciting opportunity awaits experienced Registered Nurses with a passion for delivering patient-centered care in a home-based setting. As a Home Health RN, you will play a pivotal role in supporting individuals on their journey to wellness, providing skilled nursing care directly in patients' homes and ensuring optimal care continuity and outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive assessments (OASIS) and develop individual care plans for home health patients.
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, quickly identify changes in condition, and communicate with interdisciplinary care teams and physicians as needed.
- Educate patients and families on disease processes, medication regimens, and preventive care strategies.
- Maintain up-to-date documentation using Home Care Home Base (HCHB).
- Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate high-quality care.
- Participate actively in case conferences, staff meetings, and ongoing education.
- Adhere to all regulatory and best practice standards in patient care and recordkeeping.
Qualifications:
- Active RN license in good standing for the state of practice.
- At least 1 year of recent home health nursing experience (field work preferred).
- Proficient with Home Care Home Base (HCHB) and OASIS assessments.
- Strong assessment, clinical, and critical thinking skills.
- Reliable transportation, valid drivers license, and current auto insurance.
- Excellent communication, organization, and time management abilities.
- BSN preferred; prior experience managing 2532 points per week a plus.
- Knowledge of Medicare and home health compliance preferred.
