Job Description
Deliver skilled, compassionate nursing care where it matters mostright in patients homes. Experienced Registered Nurses who are organized, self-motivated, and value clinical excellence are needed for a contract Home Health role. Youll play a critical part in continuity of care, individualized care planning, and education for patients and families, all while collaborating with interdisciplinary teams for optimal outcomes.
Schedule/Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, with daily travel within an assigned territory (mileage reimbursement provided)
- Caseload: 2532 points weekly, typically 35 patient visits per day
Key Responsibilities:
- Conduct OASIS assessments and develop individualized care plans
- Administer nursing care: wound management, medication administration, IV therapy, chronic disease management
- Monitor patient progress, recognize changes in condition, and communicate findings to the healthcare team and physicians
- Deliver clear education to patients and families regarding disease management, preventive care, and medication
- Maintain accurate and timely documentation in Home Care Home Base (HCHB)
- Coordinate with therapists, medical social workers, and home health aides
- Participate in case conferences, staff meetings, and ongoing professional development
- Comply with all regulatory standards and agency protocols
Required Qualifications:
- Active RN license (in good standing for the state)
- At least 1 year of recent home health experience (field experience highly preferred)
- Proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience
- Strong assessment, clinical, and critical thinking abilities
- Reliable transportation, valid drivers license, auto insurance
- Exceptional communication and organization skills; autonomous fieldwork capability
Preferred Credentials:
- BSN
- Experience with 2532 point caseloads per week
- Familiarity with Medicare and home health compliance
