Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care directly to patients in their homes. This role requires a self-motivated professional capable of working independently while upholding clinical excellence and effective patient communication. You will conduct in-depth assessments, develop tailored care plans, and coordinate with interdisciplinary teams to ensure positive patient outcomes. Providing patient and family education, monitoring progress, and adhering to physician directives are essential components of the position.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
  • Evaluate patient progress, identify condition changes, and communicate with care team members and physicians promptly.
  • Educate patients and families on disease processes, treatments, medications, and preventive care.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and home health aides to coordinate comprehensive care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Ensure compliance with all regulatory standards governing home health care and documentation.

Qualifications and Desired Experience:

  • Active RN License in good standing for the state of practice.
  • Minimum of 1 year recent field experience in Home Health nursing.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, current drivers license, and auto insurance.
  • Excellent communication, time management, and organizational skills.
  • BSN preferred.
  • Familiarity with Medicare and home health compliance standards.
  • Experience managing 2532 points per week preferred.

Job Details:

  • Location: Woodbridge, VA area (daily travel to patient homes within assigned territory; mileage reimbursed).
  • Schedule: Monday through Friday, 8:00 AM 5:00 PM.
  • Caseload: Typically 35 patient visits daily, totaling 2532 points weekly.
  • Contract Length: Full-time, 13-week contract.
