Provide compassionate, patient-centered nursing care and make a meaningful impact as a Home Health Registered Nurse. In this contract position, you'll deliver skilled clinical services, comprehensive assessments, and care coordination to patients in their own homes, promoting positive outcomes and quality of life. The role requires independence, strong critical thinking, and excellent clinical expertise.
Schedule and Hours:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Field-based, with daily travel within your assigned territory (mileage reimbursement included)
Key Responsibilities:
- Conduct thorough OASIS assessments and develop individualized home health care plans.
- Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, detect changes in condition, and effectively communicate with physicians and the care team.
- Educate patients and their families about diagnoses, treatments, medications, and preventive health strategies.
- Document care accurately and promptly using Home Care Home Base (HCHB).
- Collaborate closely with physical and occupational therapists, social workers, and home health aides.
- Participate in interdisciplinary case conferences, staff meetings, and ongoing education.
- Ensure compliance with all regulatory and agency standards.
Required Qualifications:
- Active RN license (in good standing) in the state of practice
- At least one year of recent home health (field) experience
- Proficiency with Home Care Home Base (HCHB)
- OASIS assessment experience
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to work autonomously and manage a flexible schedule
Preferred:
- BSN degree
- Experience managing a 2532 point caseload
- Familiarity with Medicare and home health regulations
