An exciting opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care to patients in their homes. This position involves working autonomously and collaboratively to support patient health goals within a structured, supportive environment.

Schedule and Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Complete comprehensive OASIS assessments to develop individualized care plans tailored to patient needs.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and management of chronic conditions.
• Monitor patient progress closely, promptly addressing any health status changes and coordinating with healthcare teams.
• Educate patients and their families on treatment regimens, disease processes, medications, and preventive health strategies.
• Maintain timely, accurate electronic documentation using Home Care Home Base (HCHB).
• Work collaboratively with therapists, social workers, and aides to ensure continuity and quality of care.
• Attend team meetings, case conferences, and participate in ongoing education as required.
• Uphold regulatory guidelines and standards for documentation and patient care.

Qualifications:

• Active RN license (in good standing) required.
• Minimum 1 year of recent Home Health experience, preferably in the field.
• Proficient with Home Care Home Base (HCHB) and OASIS assessments (required).
• Strong assessment, organizational, and critical thinking abilities.
• Reliable transportation, current drivers license, and active auto insurance.
• Self-motivated and able to handle a flexible, field-based schedule.
• BSN degree preferred.
• Familiarity with managing caseloads of 2532 points per week and Medicare compliance a plus.

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
