Job Description

An exciting opportunity awaits for an experienced Registered Nurse skilled in Home Health to provide patient-focused care in a field-based setting. This contract position offers you the chance to make a lasting difference in the community, delivering quality healthcare to patients in the comfort of their own homes.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Perform thorough OASIS assessments and create tailored care plans.
  • Deliver skilled nursing interventions, such as wound care, IV therapy, chronic disease management, and medication administration.
  • Monitor patient conditions, evaluate progress, and coordinate updates with physicians and care teams.
  • Educate patients and families on treatment protocols, disease management, medication regimens, and preventive care.
  • Maintain electronic documentation using Home Care Home Base (HCHB) and uphold agency and regulatory standards.
  • Collaborate closely with interdisciplinary cliniciansincluding therapists, social workers, and home health aidesto ensure comprehensive care.
  • Attend case conferences, staff meetings, and participate in relevant continuing education.
  • Travel daily to patient homes within your assigned territory (mileage reimbursement provided).

Required Qualifications:

  • Active RN license in good standing for the state of practice
  • A minimum of one year of recent field-based Home Health experience
  • Proficiency in Home Care Home Base (HCHB) and OASIS is essential
  • Strong clinical judgment, communication skills, and organizational abilities
  • Ability to work independently, equipped with reliable transportation, current drivers license, and auto insurance

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Experience managing a caseload of 2532 points weekly
  • Familiarity with Medicare and home health compliance standards

$2,490 - $2,634/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
